Founded in 1990, Rainbow Community Care has provided services that support patients and families in Wisconsin throughout the entire journey of aging, serious illness and loss, caring for people wherever they call home. For more than 35 years, Rainbow Community Care has been highly regarded for excellence in hospice care, palliative care, advance care planning, grief support, and volunteer programs.
From the outset, our shared goal was to produce a personalized hospice guide that was not only visually appealing and easy to navigate, but also deeply informative and supportive for the families who would rely on it. Offering a broad spectrum of compassionate programs, from Advance Care Planning and Supportive Care Management to Grief Support and Friends In Action, our guides encompass everything that Rainbow has to offer for patients, families, and caregivers navigating end-of-life care. With a purpose-built 8-bed Inpatient Center recently constructed in Johnson Creek, a dedicated interdisciplinary care team, and a robust network of support services, Rainbow Community Care ensures that patients and families receive the highest level of care and dignity in life’s final chapter.
As a member of the National Partnership for Healthcare and Hospice Innovation (NPHI), Rainbow Community Care were eligible to include in their guide our exclusive Innovation Lab inserts, showcasing NPHI's Dementia Care guide, Advanced Lung Care guide, Advanced Cardiac Care guide and Serving Those Who Served Us guide, each designed and published by Partner Plus Media in collaboration with NPHI. This integration allowed Rainbow to extend valuable, specialized knowledge to patients and families beyond their core programs.
We feel very privileged to be let into the worlds of our partner hospices and were delighted by the level of dedication and enthusiasm shown by Rainbow Community Care in personalizing their guide. The inclusion of a selection of newspaper articles highlighting human-interest stories related to Rainbow Hospice and their patients, for example, resulted in a uniquely personal experience for readers.
The new hospice guide has strengthened Rainbow’s ability to educate the community, empower families to make informed decisions, and build trust in their care services. As a result of our partnership, patients and families in Wisconsin have a single, comprehensive resource that combines program details, practical information, and emotional support.
Our first year with Rainbow Community Care has laid the foundation for a lasting partnership. With their commitment to innovation and our expertise in creating engaging, impactful guides, we look forward to producing future editions that continue to reflect Rainbow’s mission, helping individuals and families find comfort, dignity, and peace.
It has been a true pleasure working with the Partner Plus Media team to write, design, and publish Caring Choices: A Roadmap for Enhancing Life's Journey, Rainbow Community Care's custom resource guide for the people of rural Southern Wisconsin! Underwritten by twenty-one close business partners, this beautiful book has enabled us to explain how our various programs enrich lives throughout aging, illness, and loss. Over the course of the past year, we have distributed 1,000 physical copies of the publication to area churches, clinics, hospitals, nursing homes, assisted living facilities, public libraries, and senior citizen centers, and reached thousands more by promoting the digital version via our website, social media, e-blasts and newspaper ads. As Rainbow continues to innovate and grow, we look forward to collaborating with Partner Plus Media to produce updated versions of Caring Choices!
