Trustbridge

Since 1978, Hospice of Palm Beach County and Hospice by the Sea have cared for more than 200,000 families across South Florida. Today, these renowned providers are united under the umbrella of Trustbridge, a nonprofit organization offering 24/7 support for families facing serious illness. We partnered with Trustbridge to create three separate hospice guides, tailored to and to be distributed from each of their three Florida locations: Hospice of West Palm Beach, Hospice by the Sea in Boca Raton, and Hospice of Broward County in Fort Lauderdale.

In addition to hospice care, Trustbridge offers palliative medicine, caregiver support, and bereavement programs, and has been continually recognized for its commitment to excellence including the receipt of The Joint Commission’s Gold Seal of Approval® for Home Care Accreditation, and so many more. Trustbridge is also among just 4% of hospices nationwide that provide extraordinary comfort treatments such as radiation, chemotherapy, IV medications, and blood transfusions, ensuring patients have access to the most comprehensive care possible.

Our collective aim was to create region-specific guides that would reflect the unique offerings of each location while maintaining Trustbridge’s unified brand identity and mission of compassionate care across Florida. Trustbridge Hospice Care provides an array of programs, from music and pet therapy through to bereavement, including open support groups, loss-specific support groups, workshops and their Sea Star Children’s Programs for children and teens who have experienced the death of a loved one.

Partner Plus Media managed the full production process, from the initial planning and content collaboration, to the design of the guides, to the print and delivery of the guides to their three locations. These Trustbridge hospice guides place essential information at the fingertips of patients, their families and caregivers, providing clear and accessible information to help them navigate end-of-life decisions, connect with support services, and understand the breadth of care available to them. By producing three distinct yet interconnected guides, Trustbridge can serve its diverse communities more effectively, ensuring that patients from West Palm Beach to Fort Lauderdale receive not only exceptional medical care, but also emotional and spiritual support tailored to their needs.
